We took him to the vet and they gave him medicine to get rid of them, we are on week 4 and he still has them. What do I do?

In very young puppies, the product they use called Pyrantel only kills the worms that are alive in the intestinal track, not migrating larvae. It is used because it is very safe for young dogs. You just have to use it repeatedly.
Since your dog is 2mo old, he may be old enough for your vet to feel safe giving him a different wormer that will kill migrating larvae, such as the ivermectin someone else suggested. And he might be eligible to have a different type of wormer that will kill different types of worms. Call your vet and see.
It could simply be that your vet is using the safest wormer (pyrantel) for him because he is small. If that is the case, you have to do it over and over.
Back to the vet and clean up all the old poop in the yard

If your dog is not a collie breed then try Ivomectin. It is very strong and should do the trick.
have you called the vet back ? did you clean up the yard 100%. is the dog eating it’s own poop ?
worms can come from fleas. treat for fleas, also.

Possibly you need to try a different vet? 4 weeks should be more than enough to rid of worms. Of course, if you are not cleaning up his feces every time he goes, his chances of getting them back are much higher.
Depends what kind of worms. take him back to the vet for another treatment, they will know which meds to give him. If they are rouns then it can sometimes take a few treatments. Make sure to clean up all poo in the yard. The eggs are still in the poop.

get a pouch of red man chewing tobacco and give him a decent size pinch. it sounds gross and the dog may puke but it works
worm treatment needs to be repeated at 2 week intervals until gone, only a certain type of tape worm is from fleas, if there are no fleas, don’t treat a young pup for them. Keeping the pups area dry, clean and poop free is essential, worms leave eggs in the poop which then reinfest the dog.
Nobody mentioned this yet. Bring a stool sample to the vet. There are several types of worms. The worms you see may not be the worms he was treated for. The only way a vet can know what wormer to give him is to analyze a stool sample.
What kind of worms are these? If they are round worms, sometimes two-three treatments are needed to get the hatching larva. If tape worms, you will need to also make sure your puppy doesn’t have fleas, as they are the catalyst for tape worms.
Note: Please don’t give your puppy tabacco, as it can kill him.

Take him back to the vet. Take a stool sample along there may be worms that were not visible before or he may need a stronger dose of medicine.
